Crescent Health Care accepts Medicare, private payment, long-term care insurance as well as other insurances, and Medicaid. Information may be obtained through our Admissions office.
Crescent offers various levels of skilled nursing and rehabilitative care. There are registered nurses and licensed practical nurses as well as other nursing personnel on duty 24 hours a day to provide you with your nursing needs.
Our Case Managers are assigned to each resident to assist day to day. Care conferences are held upon admission and every 6 months so that the nursing department, activities department, dieticians, social services, and physical therapy areas may discuss your care. You and your personal representative are invited to attend this conference.
Our Director of Nursing and all department managers are available if you have a problem or concern about your care. Do not hesitate to let us know if you have a concern. If you have a personal or family problem you would like to discuss, please arrange a confidential meeting with one of our social workers.
Arrangements are made by the nursing department for physical therapy, speech therapy, occupational therapy, or respiratory therapy and other specialized services as ordered by your physician.
We offer semi-private 2 bed rooms, 2 bed suites and 3 bed rooms, all with either private or adjoining bathrooms. All rooms have telephone and cable access.
The quality of the food is excellent. In addition to our 3 meals a day, in-between meal snacks are also offered. Our dining staff consists of qualified preparation and service staff. We have an experienced cooking staff who cater to each resident’s nutritional needs as approved by our Registered Dietician.
Yes, we welcome you to have guests join you and appreciate advanced notice. There is a charge for guest meals.
Yes, all exterior doors are locked and access to the outside is only through the use of a key pad.
All residents must be under the care of a physician. This physician may be their own personal doctor or they may use Crescent’s medical director. Our medical director, along with his associates, provide most medical services at the facility. Out of facility physician appointments, such as dental or podiatry visits, are arranged by the nursing department. Should hospitalization be required, all arrangements for transfer to the hospital will also be made by the nursing department in cooperation with the resident’s physician.
No, family and friends are encouraged to visit at their leisure. However, there are times when visitation may be limited based on special circumstances (roommates, illness, etc).
